These packages of care provide choice and flexibility in the way services are provided. They include:
Support services – such as washing and ironing, house cleaning, gardening, basic home maintenance, home modifications related to care needs and transport to help with shopping, visiting the doctor or attending social activities
Personal care – such as help with showering or bathing, dressing and mobility and medication supervision
Nursing – allied health and other clinical services – hearing and vision services, care coordination and case management
Neighbourcare provides all four levels of care and the package level is determined through an assessment by the Aged Care Assessment Team (ACAT).
As a Home Care Package consumer you may be eligible to receive a government-funded subsidy plus you will be required to make a co-payment to support your selected services.
These amounts will be determined on the outcome of your Income and Asset assessment.
Recently there have been letters sent out offering eligible people Home Care Packages.
