From the first residential aged care facility opened in 1988 the NCCCA has expanded to include Jacaranda Grove and Abernethy Home located at 2 Mount View Road, Cessnock.
NCCCA also provides community home care services that allow you to stay living in your own home. Services are flexible and can be tailored to meet your individual needs.
NCCCA offers:
- Home Care Package (HCP)
- Commonwealth Home Support Program CHSP.
HCP
Home Care Packages are provided on a consumer directed care model of service delivery design to give more choice and flexibility to the consumer enabling the consumer to have more control over the types of care and services they seek.
It provides the greatest level of support and enables you to create a tailored, personalised care plan that will work for you.
This is integrated into your existing lifestyle and includes a focus on individual strengths and abilities, which means if you need some support around your home with day-to-day tasks we can help support you.
If you have more complex health needs or develop an illness or injury, we’ll include our specialist registered nurse to the home nursing, clinical or therapy services in your care plan. Services can be provided privately or through Government funded support.
CHSP
A Commonwealth home support package (CHSP) is for over 65s who just need a little support to stay active and social or could do with a little help around the home.
These services are subsidized by the Government and can include a range of domestic assistance, personal care, shopping and transport options.
Additionally, NCCCA can supply meals on wheels services with delivery extending throughout Cessnock, Kurri Kurri and Branxton by dedicated staff and community volunteers to make sure you receive nutritionally balanced meals through Meals on Wheels NSW.
Whether you require a little assistance or a lot NCCCA have options and services that can best meet your needs.
Government subsidies are available for home care services depending on your personal circumstances and your level of care required.
To receive care support, you will first need to have a free assessment.
The initial stage is conducted through a screening of eligibility through “My Aged Care” and transferred to either the Aged Care Assessment Team (ACAT) if your care needs are deemed eligible for a Home Care Package or the Regional Assessment Team (RAS) if your care needs are considered for the Commonwealth Home Support Program,, assessments based on your level of care needs and the type of care that is right for you.
Waiting times for an assessment vary depending on your type of assessment you need.
Once an assessment is booked the assessor will also talk to you about what makes life enjoyable for you, your individual needs and your goals which make life meaningful for you.
You will also be asked who you would like to provide your services. Be sure to mention NCCCA so that following your assessment we will receive your referral and we will be in touch to arrange the care your need.
It is easy to arrange an assessment either by speaking with your GP to arrange an assessment, by contacting MyAgedCare(www. myagedcare.gov.au) or telephone 1800 200 422 or with your permission the friendly staff at NCCCA can assist in arranging an appointment for you.
NCCCA can also provide services for your NDIS package. Whether its domestic assistance and personal care through to transport for social events and appointments there are a range of services that can be customised to meet the daily needs of your care and connectivity.
Later in the year NCCCA will open its community support centre on Wollombi Road Cessnock at the site of the former PCYC.
Refurbishment is ongoing with the site set to become a one-stop shop designed to fulfil all the needs of the community requiring assistive services while remaining in the home – HCP and CHSP services, meals on wheels, community transport, NDIS services and space for community forums on the care needs within the LGA.
