The Westpac Rescue Helicopter was called to assist at the scene of the accident on Wollombi Road just before midday.
The aircraft medical team treated the 20-year-old woman for multiple injuries at the scene. She was flown to the John Hunter Hospital in a serious condition.
Three other patients were taken to hospital by road ambulance.
Police are investigating the circumstances of the crash.
