Safety was by far the biggest worry for motorists with Hermitage Road in Pokolbin, Redgum Road in Paxton, the Hunter Expressway in Loxford and Main Road at Greta called out.
Wollombi Road was also identified for its poor condition and safety concerns, while O'Connors Road in Pokolbin and Nulkaba scored poorly for its condition, safety and congestion.
There were more than 300 votes for roads in the Cessnock Local Government Area and 10,000 votes for the Hunter region. The NRMA will use the findings, which were released on Monday, to lobby the state and federal government for more road funding.
NRMA spokesperson Peter Khoury said policies around roads and public transport had to be a priority while cities and towns continued to expand.
"These survey results are telling," Mr Khoury said.
"Where money is spent we see improvements, so I'm pleased to see both sides of politics pledging billions of dollars to our roads in the lead up to the election."
The survey found the New England Highway was the worst road in the Hunter.
The arterial road took out the top spot in the survey because of its congestion and attracted 209 votes from motorists.
Traffic chaos on the highway at Rutherford, South Maitland and Maitland were among the congestion hotspots raised.
Nelson Bay Road was named the second most congested road in the Hunter, the Pacific Highway at Hexham was third and Newcastle Road was fourth. Dungog's Dowling Street took out fifth place for its poor condition.
